How to say "million-first" in Italian?

I can't find any examples online or in textbooks of how to say more complex ordinals higher than "millionth".
It seems to me there could be four ways to say "million-first":
1) milionesimo primo
2) milionesimo e primo
3) un milione e primo
4) un milione primo

I've also seen "milionunesimo" but I doubt that this is correct.
If it is correct for 1000001st, then would 1000008th be "milionottesimo"?
In what situations would "milion" form a prefix to a larger word, instead of just having "milionesimo" followed by a second word?

babel fish gives "milonesimo" but I reccomend you check that with a native.
